Senior Support Services will perform a comprehensive in-home assessment which evaluates the following elements: Functional Status, Physical Health, Cognition, Psychosocial, Environmental, Emotional and Behavioral status. Once the assessment is completed, Senior Support Services will provide the client with a thorough recommendations report based on the results and findings of the assessment.
Once the Plan of Care is in place, Senior Support Services will refer its clients to resources within the community that provide the services that the client has decided will work best for him/her and allow him/her to be as independent as possible. Senior Support Services will help coordinate the commencement of services and will be available to monitor the services to ensure that quality care is maintained.
Senior Support Services will be available to families (both locally and those attempting to provide long distance care) and their loved ones as a consultant to ask questions, discuss ideas and plans, or simply be a listening ear.
Weighing your options? Senior Support Services is an unbiased, educated and experienced Geriatric Care Management business which provides a Geriatric Care Manager who works for the client, not another entity. Therefore, Senior Support Services will work in the client's best interest and advocate for that client.
If placement in a long term care facility is necessary, Senior Support Services will help locate a facility that provides the right fit and assist clients through the entire transition process.
Senior Support Services will determine and apply an appropriate timetable for reassessment of the client to ensure that the original services are still sufficiently meeting the needs of the client. If it is found that changes need to be made in the plan of care, Senior Support Services will make those changes and arrange different/new services as needed for the client.
